New Patients

Providing excellent dental care for you and your family is ALWAYS our first priority. Collecting proper information will allow our team to provide you with the most comprehensive care possible in the most efficient manner possible. Prior to your first appointment at Eastview Family Dentistry, a member of our team will initiate your digital file by collecting your demographic (address, phone number, birth date, etc) and insurance information. Please have your insurance card available during that phone consultation in order to provide sufficient information. PRIOR TO YOUR APPOINTMENT, PLEASE READ OUR OFFICE POLICIES (see below). Please bring your insurance card, a form of identification, and a list of all current medications and allergies to your first appointment. Upon arrival at the office, we will provide you with a private digital work station to verify your demographic information and to complete your medical history. You will also sign receipt of our policies and a consent form for treatment. Most first time patients to Eastview Family Dentistry will need 15-30 minutes to complete all forms. Returning the appropriate RECORDS RELEASE FORM as soon as possible and reading the OFFICE POLICIES prior to your first visit can also expedite this process.
