- Careington (includes Anthem BCBS, AXA)
- Dentemax (includes AlwaysCare, Beam Dental, Colonial Life, Mutual of Omaha, Renaissance Dental, Starmount, TruAssure, Unum)
- G.E.H.A.
- Guardian
- Humana
- Lincoln Financial Insurance Company
- NovaNet Inc. (includes Aflac PPO Dental, CareFirst BCBS, Equitable)
- Principal Financial Group
- Sun Life Financial, Inc.
- Some United Concordia policies
- United Healthcare
- UMR
- Metlife PPO
- VA Community Care – for veterans that are 100% disabled; contact VA Community Care at (205) 933-8101 and request for your dental services to be provided by Eastview Family Dentistry. These visits are on referrals by community care only.
**Eastview Family Dentistry is not in network with any HMO plans**

Although Drs. Congo and Dean may not be in contractual agreement with your insurance company, we want to help you receive the maximum reimbursement to which you are entitled.
